Starting Monday, December 31, 2007, Holiday MILLIONAIRE RAFFLE™ prizes of $100,000 and $10,000 may be claimed at any Florida Lottery office. However, the $1 million prizes must be claimed at Florida Lottery headquarters in Tallahassee. All prizes must be claimed by June 26, 2008. You must have a Holiday MILLIONAIRE RAFFLE ticket to claim a prize.
Each Holiday MILLIONAIRE RAFFLE winner must present his or her official Holiday MILLIONAIRE RAFFLE ticket, a completed
Winner Claim Form*, an IRS W-9 Form* and an acceptable form of identification to a Lottery office for payment. Federal income tax withholding will be deducted from all cash prizes.
Holiday MILLIONAIRE RAFFLE tickets must be claimed within 180 days of the draw date, and may be validated at any Florida Lottery retailer. Holiday MILLIONAIRE RAFFLE tickets may be mailed for redemption to: Florida Lottery, Claims Processing, 250 Marriott Drive, Tallahassee, FL 32399-9939. The risk of mailing tickets remains with the player.
$50 Instant-Win Prizes
$50 instant-win prizes may be redeemed at any authorized Florida Lottery retailer or at a Florida Lottery office. A valid Cash Prize Coupon must be presented for payment, but you must keep your Holiday MILLIONAIRE RAFFLE ticket for the December 29th drawing. Cash Prize Coupons must be claimed by February 27, 2008.
