Some of the forms may be utilized during a sealed competitive solicitation process, to update information during a contract term if a vendor is awarded a contract, or for a contractor to report minority business participation information.
Certification of a Drug-Free Work Place The Florida Lottery encourages all business entities to adopt policies regarding substance abuse in the workplace. Submission of this form in a sealed competitive solicitation process by a Vendor may also be used to break a tie in the event of tie bids or scores.
Conflict of Interest & Disclosure - Information required to be disclosed to identify potential Conflicts of Interest that a vendor have in conducting business with the Florida Lottery. This information is required to be submitted by all vendors who are responding to a sealed competitive solicitation. A vendor who is contracted with the Florida Lottery must update this information as necessary during the contract term.
Disclosure Affidavit & Vendor Personal Profile Form - Information which may be required in procurements where the Florida Lottery desires or is required to investigate the background of vendors and it's officers. A vendor who is contracted with the Florida Lottery must update their registration information as necessary during the contract term.
90 Day Spending Plan Form - Information which may be required of a Florida Lottery Contractor pertaining to purchases to be made in the near future. This information is provided to the Office of Supplier Diversity and posted on their website in an effort to increase the involvement of minority businesses in the State's procurement processes. The information which is provided in this form should be as accurate as possible but is considered for informational purposes only and is not a commitment that such purchases will be made.
Minority Business Utilization Report - Information which may be required of a Florida Lottery Contractor pertaining to actual expenditures made with Minority Businesses during the preceding month. Information provided in this report is used to track a Contractor's performance in possible contractual commitments.
